How do I get my 1095 tax form?

The 1095 form is a health insurance tax form which reports the type of coverage you had, dependents covered by your insurance policy, and the period of coverage for the prior year. This form is used to verify on your tax return that you and your dependents have at least minimum qualifying health insurance coverage.

There are several different 1095s. Use the table below to identify where to find it, and when you are likely to get it.

All 1095 forms are sent by the insurance carrier that you had for the year in question. HSAInsurance.com does NOT create or distribute these forms.

1095-A A 1095-A form includes details about the Marketplace insurance you and any household members had for the previous year. You’ll need it to complete Form 8962, Premium Tax Credit.

People who have enrolled in a plan through a state or federal marketplace. Early February

1095-B 1095-B is a health insurance tax form which reports the type of coverage you have, dependents covered by your insurance policy, and the period of coverage for the prior year. This form is used to verify on your tax return that you and your dependents have at least minimum qualifying health insurance coverage. If you had a break in health care coverage for the tax year, you may have to pay an individual shared responsibility payment, also known as a tax penalty.

People who have Individual or small employer(less than 50 employees) health insurance coverage.

Mid March




1095-C Form 1095-C is a health insurance tax form which reports the type of coverage you have, dependents covered by your insurance policy, and the period of coverage for the prior year. This form is used to verify on your tax return that you and your dependents have at least minimum qualifying health insurance coverage. If you had a break in health care coverage for the tax year, you may have to pay an individual shared responsibility payment, also known as a tax penalty.

Employees of large group (50+) employers. Mid March

1099-HC The 1099-HC form is a Massachusetts document which serves as proof of health insurance coverage for MA adult residents.  Every MA resident who has insurance will receive an annual 1099-HC form which is completed and provided by your insurance carrier.
